Notice of Privacy Practices

Abby’s Learning Center, LLC is dedicated to keeping your information private and secure. This notice describes how your information may be used and disclosed, how you may access your information, and how Abby’s will protect your information. Please review this document carefully. You will be asked to sign an acknowledgment that you understand these practices.

1 - Abby’s is required by law to maintain the privacy and security of patient health information. All providers, staff, and volunteers at Abby’s will adhere to HIPPA requirements. You will be notified if a breach occurs or if your health information has been compromised in any way.

2 - Abby’s will never use or share your information except for the purposes described in this notice or with your explicit written permission/request. In the case that you give permission to share your information, you may revoke the permission at any time. Abby’s may use or share your information for the following purposes:

• Collaboration with multiple providers involved in your care for assessment or treatment purposes.

• As required by your insurance provider or other payor to obtain payment for your services.

• To improve the quality of care (e.g., training new providers)

• In order to carry out health care treatment, payment, and operations functions through business associates (e.g,. audits, inspections, licensure).

• When asked by law enforcement or another government entity, during a lawsuit, or as required by law.

• To support certain research projects approved by an Institutional Review Board

3 - Mobile information will not be shared with third parties/affiliates for marketing/promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

  • You will receive a text message to confirm. “You agree to receive informational messages (appointment reminders, account notifications, etc.) from Abby’s Learning Center. Message frequency varies. Message and data rates may apply. For help, reply HELP or email us at info@abbyscenter.com. You can opt out at any time by replying STOP.”

4 - You have rights regarding your medical information. You have the following rights, provided that you make a written request.

• You can ask to see or get an electronic or paper copy of any of your or your dependents’ clinical records and other health information.

• You can ask us to correct health information you think is incorrect or incomplete.

• You can ask us to contact you in a specific way or to send mail to a different address.

• If you pay for a service or health care item out-of-pocket in full, you can ask us not to share that information with your health insurer unless a law requires us to share that information.

• You can ask for a list of the times we’ve shared your or your dependent’s health information for six years prior to the date you ask, who we shared it with, and why we shared it.

• You can ask for a paper copy of this privacy notice at any time.

• You have both the right and the choice to share certain information with family, close friends, or others involved in your care.

• If you have given someone medical power of attorney or if someone is your dependent's legal guardian, that person can exercise your rights.

5 - If you ever feel like anyone has violated your rights in any way, please contact us so that we can address the issue. You can also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by sending a letter to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-877-696-6775, or visiting: www.hhs.gov/ocr/privacy/hipaa/complaints/. Abby’s will not retaliate against you or your dependents for filing a complaint.

6 - Abby’s is required by law to provide you with this notice. Abby’s may need to change the terms of this notice. In the event of changes to this notice, we will provide notice to you when any changes take effect. The current effective date of this notice is 03-03-2025.
